Nose surgery

Introduction:

The nose surgery may have the purpose to increase it, like on the case of a saddle nose. It can be of congenital or traumatic origin and it may require bone or cartilage graft, silicone prosthesis or some material implants, like PMMA (polymethilmetacrilate). In the case of the negroid nose, with a thick skin, we reduce the nose wings and thin the nasal tip.

In the case of a nose with augmented hunch, it is necessary to remove fragments of bone and cartilage, most of the times, by fracture.

Indication:

The indication for rhinoplasty is only made in a consultation. The clinic history must be detailed, to know about possible trauma in the past and if it involves a previous nasal surgery. If you had a previous surgery on the nose, it is only possible to operate again, after a year.

It is necessary to know if the respiration is normal, if there are obstructions, occasional or permanent.

Dc. Yhelda needs to know the grade of satisfaction with the shape of your present nose, what burdens you more. The wish of the patient must be very clear. Sometimes he longs for someone else’s nose and this is impossible to fulfill.

Dc Yhelda reminds you that she may work with proportions; this means she may only obtain a nose proportional to your face.

With graphic computation we may try to show what can be changed in your nose and if the change will be favorable to your face.

Dc. Yhelda reminds you that the computer may help the client to decide for or against the operation.

How is this surgery done?

It may be by an open or closed way. The incision is made in the columela (central part of the nose), internally, if the surgery is to be closed. If it is to be open, we make a V shape incision in the central part of the columela. We detach the tissue on the nasal bones and wing-cartilages. The advantage of the open surgery is to visualize all structures and work more safely.
The amount of bone and cartilage to be removed is determined with precision when the nasal wings are enlarged. We make the incision on the junction of the nose with the face, resulting in a scar of good quality, since it stays in a natural ply of the face.

What type of anesthesia is it used?

On most of our cases, without fracture, we use local anesthesia, with sedation. With fracture of the nose bones, we use general anesthesia and the patient stays in the hospital or clinic, for the night.

How long does the operation last?

Approximately one hour.

What happens after the surgery?

The whole face gets swollen for approximately 30 days. On the first 30 days, this edema is clearly apparent. It lasts for another 6/12 months, but much less apparent. During the first 24 hours the respiration is done through the mouth, since the nostrils stay obstructed by tampons. This circumstance may also last for 48 or 72 hours, if necessary. For 15 days you use a hard dressing on the nose, to keep it immobile.

We recommend the use of oral antibiotics and an anti-inflammatory, for one week, and the toilet of the nostrils with saline solution, taking off the crusts that intensely form on the first week, after the operation.

Avoid physical exercises for one month and intensive sun exposure for 6 months. The eyes get reddish and swollen, just like on a face trauma, for 15 days, approximately.

You must remember that the final result of this surgery can be seen only after a year. The clients usually do not complain of any pain. They just feel some discomfort, for the time the nostrils remain with tampons.

What type of scars shall I have?

It is impossible to precise your type of cicatrisation. In this surgery the scar is usually satisfactory because it stays on natural plies of the body. On the first 3 months, the scar is reddish and very apparent, but one year later, it becomes almost imperceptible.

What may go wrong?

When we go out of our home we begin to take risks, the same with any surgical treatment.

If your operation is done by a specialist with large experience (more than 50 rhinoplasties), there are much more chances of success. Complications of the rhinoplasty are not frequent, but individual reactions may require a later correction.

It may also occur the fact that the patient is not satisfied with the new shape of his nose. This is a more difficult problem to solve because it is a subjective question. We remind you the importance of letting the doctor know exactly what you want and understand your wishes.

In a survey about plastic surgery medical processes, it was shown that on most cases, the point was dissatisfaction of the client with the results, and not medical practice.

When may I resume my regular activities?

Depending on your profession, if you don’t want any constraint in your contacts with the public, the ideal time will be one month. If necessary, 15 days will also do. Physical exercises should be postponed for one month, and direct exposure to the sun, only after 6 months.
